An unannounced monitoring inspection was conducted today.
There were 12 preschool children (ages 3-5yrs) present with two classroom staff.
The children were playing in the gym and eating a snack.
Three child records were reviewed.

Based on a tour of the playground, the facility failed to ensure that all areas are kept in safe condition.
Evidence:
1. On the plastic climber, there are 1/4 inch gaps where the blue slides meet the decks. These gaps present entanglement points.
2. There are four metal stakes protruding from the retaining plastic guards. These areas present protrusion hazards.

Plan of Correction: The facility manager will fill in gaps on the climber. Children will not use the climber until it is fixed.
