Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 3/17/2021 and concluded on 3/19/2021. The director was contacted by telephone to initiate the inspection. There were 7 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records, 2 staff records, and 1 volunteer record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, the center did not obtain the results of a follow-up tuberculosis (TB) test or screening for all staff completed at least every two years from the date of the first initial screening or testing.

Evidence: Staff #1's most recent TB testing on file was completed 12/13/18.
Plan of Correction: Staff #1 will receive TB testing on 3/30/21. Results will be sent to licensing representative by 4/7/21.

Based on record review, the center did not obtain results of a criminal history record information check and a sex offender registry check for all states where staff have resided in the last 5 years.

Evidence: Staff #2 (Date of Hire: 7/1/2020) has resided outside the state of Virginia within the last 5 years and does not have a Pennsylvania criminal history record check or a sex offender registry check on file.
Plan of Correction: Additional background checks were completed on 3/17/21 and results were sent to licensing representative.
