Inspector Notes

An unannounced monitoring inspection was conducted 2/24/2023 from9:45am through 10:40am. There were 6 children directly supervised by 1 staff and the classroom was in ratio. The physical plant, outdoor playground area, programming, 2 staff records, 5 children?s records, emergency drills, injury reports, emergency supplies, and policies were inspected. There were reportedly no medications on site at the time of inspection. Children were observed participating in a water table activity with shaving cream, completing a group matching activity, practicing spelling their names, practicing counting, and learning about cleaning teeth and preventing cavities. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not obtain a physical examination for each enrolled child that was within 12 months prior to attendance for children two years of age through five years of age.

Evidence: Child #1 (Date of Enrollment: 9/16/2022) had a physical examination on file that was completed 7/30/2020.
Plan of Correction: The family was notified February 24th about the missing physical exam for their child's file.

Mom informed that a doctor's appointment has been scheduled.

Once we receive it, we'll be adding to the child's file.
