Inspector Notes
An unannounced, on-site monitoring inspection was conducted on 9/12/24. The on-site inspection began at 10:15am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 10 children present and two staff. The inspector reviewed five children?s records and three staff records on-site and electronically on 9/13/24. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/2/24. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-245-L
There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.
On 9/12/24 there was no staff on duty with daily health observation training.
Plan of Correction: Alpha-Bet Cooperative Preschool will ensure that all staff members caring for children have completed the Daily Health Observation Training. Cooperative volunteers are also required to complete this training.

When staff are supervising children, they shall always ensure their care and protection.
On 9/12/24 the staff responsible for the administration of the center for the day reported that there was no medication on-site for children. Interview with another off-site staff later revealed that a child in care had an emergency medication on-site in their bookbag.
Plan of Correction: Alpha-Bet Cooperative Preschool will train all staff members and cooperative volunteers on which child/children have emergency medication in the locked box in the preschool office.

Medication shall be kept in a locked place using a safe locking method that prevents access by children.
A staff member reported that an emergency medication for a child was kept in the child?s bookbag in the classroom.
Plan of Correction: Alpha-Bet Cooperative Preschool will secure all emergency medications within a locked box in the school office that is not accessible by children.

Parents shall be notified when an authorization for medication expires that the medication needs to be picked up within 14 days or the parent must renew the authorization.
The medication authorization form for Child A?s emergency medication expired approximately five months prior to the inspection.
Plan of Correction: Alpha-Bet Cooperative Preschool will request an up-to-date Medication Authorization form from Child A. A log will be started to track and notify parents requiring medication to be stored/administered at school within 14 days of the current form's expiration date.

At least two staff members with current certification in cardiopulmonary resuscitation (CPR) are required to be present on the premises during the center?s hours of operation, on fieldtrips, and wherever children are in care.
On 9/12/24 there was only one staff on the premises with current CPR certification.
Plan of Correction: Alpha-Bet Cooperative Preschool will schedule training for the non-certified staff member to attend and complete a certification in cardiopulmonary resuscitation (CPR). Alpha-Bet will also make a list of Alpha-Bet families that currently have a CPR certification and will encourage others to complete the course as well.

Child records are required to contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
The record for Child A, who has been enrolled for more than seven months, did not contain a second emergency contact. The record for Child B, who has been enrolled for more than six months, did not contain the address of one emergency contact.
Plan of Correction: Alpha-Bet Cooperative Preschool will request a second emergency contact to be in the child's file for Child A and Child B and ensure that every student file contains at least 2 complete emergency contacts (name/address/phone number).

A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Child A, who has been enrolled for over seven months, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: Alpha-Bet Cooperative Preschool will request a written Allergy Action Plan, completed by the child's physician, from the parents of all children with a diagnosed food allergy.
