During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/6/25 and completed on 3/7/25. The on-site inspection began at 9:55am and ended at 11:55am. The inspector reviewed compliance in the areas listed above. There were 11 children present with one staff and one co-op parent. The inspector reviewed five children?s records and two staff records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/25/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

An updated screening for Staff #1 was due more than three weeks ago but an updated screening has not been completed yet.
Plan of Correction: The staff's TB test has been scheduled for 3/22/25.
Standard 8VAC20-780-260-B
(Repeat Violation) Annual approval from the health department is required.

The most recent health inspection on record was dated 9/21/2023.
Plan of Correction: We have left messages in order to get an inspection completed.

The center may administer prescription medications that were dispensed from a pharmacy and maintained in the original, labeled container.

The prescription medication on-site for Child D was in a plastic bag, not in the original, labeled prescription container.
Plan of Correction: The parent chose not to supply medication moving forward.

(Repeat Violation) At least two staff members with current certification in CPR are required to be present on the premises during the center?s hours of operation, on fieldtrips, and wherever children are in care.

On 3/6/25 there was only one staff on the premises with CPR certification.
Plan of Correction: Our secretary has been staying in the building on Tuesday and Thursdays as she can work from her computer and is CPR and MAT certified.

(Repeat Violation) A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child D, who has been enrolled for more than 13 months, has a diagnosed food allergy and did not have a written care plan in their file.
