Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review.

A monitoring inspection conducted on December 10, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 31 children present on the date of inspection with adequate staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, two of three children's files did not contain all required information
Evidence:
1. Child 1's record did not contain complete street addresses for two designated people to contact in an emergency if a parent cannot be reached.
2. Child 3's record did not contain a complete street address for one of two designated people to contact in an emergency if a parent cannot be reached.
Plan of Correction: Director and Admin. Ast. will check all the emergency forms. We will add any missing addresses for the emergency contacts.
