During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/08/2025, as a part of the licensure period. The on-site inspection began at 9:40am and ended at 11:38am. The inspector reviewed compliance in the areas listed above. There were 31 children present, 5 staff and 4 volunteers. The inspector reviewed 4 children?s records, 3 staff records, 3 board member records and 4 volunteer records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to have board members undergo a background check every five years.

The most recent central registry check for board member #2 is overdue by a two months and 18 days.
Plan of Correction: We will send the request today.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #3 TB was dated 3 months prior to date of employment and has been employed for over 4 months.
Plan of Correction: We will make sure TB tests are 30 days within date of hire.
