Inspector Notes

An unannounced code compliance inspection was initiated on 11/3/2022 and concluded on 11/3/2022 from 9:00 AM to 10:00 AM. There were 9 children present, ranging in ages from two to four, with 2 staff. Records were reviewed virtually. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, procedures and medication. A total of four children?s records and three staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, the center failed to ensure all staff have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him/her from caring for children under his/her supervision that was updated yearly.

Evidence:

1. The record for staff 1 has a sworn statement dated 9/17/21.
2. The record for staff 3 has a sworn statement dated 9/29/21.
Plan of Correction: Both staff 1 and staff 3 provided updated staff health reports on 11/4/22. In the future the director will develop a system to ensure staff health reports are updated yearly.
