Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A monitoring inspection was initiated on 09/14/2021, a virtual record review was conducted on 09/20/2021 and the virtual onsite inspection was conducted on 09/22/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 5 children, ages 3 years-4 years, and 3 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 22VAC40-185-70-A
Based on record review and discussion with staff, the center failed to ensure that staff records shall contain all of the elements as required by the standards.

Evidence: The record for Staff 2 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.
Plan of Correction: Moving forward references will be obtain prior to the hire date.
Standard 22VAC40-191-60-B
Based on record review and discussion with staff, the center failed to ensure that an employee must not be employed until the agency has the person's completed sworn statement or affirmation.

Evidence: The record for Staff 2 had a documented hire date of 09/15/2021; the date on the sworn statement was 09/16/2021.
Plan of Correction: The sworn statement will be presented during the interview and obtained prior to the hire date.
Standard 63.2(17)-1720.1-B-2
Based on record review and discussion with staff, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.

Evidence: The record for Staff 2 had a documented hire date of 09/15/2021. There was not an employment eligibility letter available for review during the 09/20/2021 record review inspection.
Plan of Correction: Moving forward, fingerprints will be obtained prior to hire.
