Inspector Notes
An unannounced monitoring inspection was conducted on August 25, 2022, from 10am to approximately 12:20pm. There were 54 children present and 10 staff members. The Inspector reviewed six children?s records and five staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, outdoor water play, story time, diapering, handwashing, and lunch. First aid kits and emergency preparedness supplies/documents were observed. No medications are currently being administered at the center. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the assistant director at the exit interview.
Violations
4Evidence:
1. In the first preschool classroom, there was a clear bottle of bleach water on the counter, disinfectant wipes, and an aerosol spray can that were in reach and accessible to children.
2. In the school and common area bathroom, there is an aerosol spray can in reach of children with a warning label stating, ?Danger, eye irritation.?
3. In the second preschool classroom, there was an unlocked cabinet of disinfectant wipes, detergent, and disinfectant spray accessible to children with warning labels stating, ?Keep out of reach of children.?
4. Staff #1 confirmed that the two preschool rooms and bathroom contained unlocked hazardous substances in reach and accessible to children.
Evidence:
1. In the infant room, the red changing pad had a tear approximately 6 inch long.
2. Staff #2 confirmed that the changing pad was torn.
Evidence:
1. In the preschool classroom, there is a bottle of insect repellent that has no label with the child?s name.
2. Staff #1 confirmed that the bottle was not labeled with the child?s name.
Evidence:
1. All four landline telephones did not have emergency numbers posted in a visible place at each telephone.
2. Staff #1 confirmed that all four landline telephones did not have emergency numbers posted close to the phone.
