During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated and completed on 5/30/25 in response to a self-report received by the licensing office on 5/8/25 relating to forbidden actions. The inspector reviewed five children?s records and eight staff records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to administration and is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/7/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's services are maintained in compliance with standards and the center's own policies and procedures.
The center's discipline policy states that "there will never be physical punishment or any disciplinary action, which involves the following: rough handling." On 5/5/25, Staff #1 did not follow the center's discipline policy. The staff member roughly handled a child in a classroom, in response to the child's behavior in the classroom. This goes against both, the center's policy and standards.
Plan of Correction: Staff #1 was terminated. Center director reviewed the agency's discipline policy with all the employees during a staff meeting.
Standard 8VAC20-780-410-1
The following actions or threats thereof are forbidden: rough handling.
On 5/5/25, Child #1 was laying on a carpet in the classroom when Staff #1 bent down to the child's level, grabbed the child by the arm, and then pushed the child back down to the carpet after the child did not follow classroom rules.
Plan of Correction: Staff #1 was terminated. Center director reviewed the agency's discipline policy with all the employees during a staff meeting.
