During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/21/25. The on-site inspection began at 7:20am and ended at 12:05pm. The inspector reviewed compliance in the areas listed above. There were 47 children present and 11 staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, a tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/7/25. A POC submitted after this date will not appear on the public website.

Repeat Violation
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
Additional immunization documentation for Child #1 were not obtained, which is required for children under two years of age.
Plan of Correction: As observed during the inspection, the child had an upcoming appointment. We now have a system in place that notifies us when children are due for immunizations, ensuring timely follow-up and improved care coordination.

Repeat Violation
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
There was a low, unlocked cabinet in a toddler classroom that contained disinfectant wipes, disinfectant spray, and window cleaner. The unlocked substances were accessible to children.
Plan of Correction: Staff involved were re-coached on chemical safety protocols and the importance of proper storage. Hazardous chemicals were immediately secured in designated storage areas.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards (at least six inches).
There was not at least six inches of resilient surfacing by a rock wall on the preschool playground.
