Inspector Notes

Thank you for your assistance during the unannounced mandated initial inspection for the relocation request. The inspection was completed on June 7, 2022 from 10:25 a.m. to 12:10 p.m. The purpose of the inspection was to gather measurements and tour the building prior to care commencing in the new location. Three children's record, two staff records, measurements, emergency care plan, evacuation maps, first aid kit, and emergency supplies were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the facility did not ensure the center's activities, services, and facilities were maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department

Evidence:
1. The license is currently issued for the facility located at 300 Grubert Avenue, Staunton, VA 24401. No children were in care at the facility on June 7, 2022.
2. The licensee submitted an initial application for an address change on May 24, 2022, and provided the last required documentation on June 7, 2022.
3. The administrator stated the center opened at 708 N. Coalter St., Staunton, VA 24401 on June 6, 2022 despite not having the license for the new location.
4. The inspector observed children in care on June 7, 2022 at 708 N. Coalter St., Staunton, VA 24401.
