Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted on 04/20/2023 from approximately 9:23 am to 12:00 pm. There were 2 children present with 1 caregiver. A licensing inspection was conducted on the same date as the subsidy supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the provider.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Violations
1Evidence: The record for the vendor documented a certificate of training for the department's annual health and safety update course dated, 2/29/2020.
During an interview with the vendor, it was acknowledged that she had not completed an updated training course.
