Inspector Notes

An unannounced renewal inspection was conducted on 5/20/2024. The inspector was on site from approximately 12:43 pm-2:40 pm. There were 3 children in care, ranging in age from 2 years to 3 and a half years with 1 caregiver supervising. The inspector reviewed compliance in the
areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of 5 child records and 1 caregiver record were reviewed.

Information gathered during the inspection determined non-compliance with an applicable standard or law and a violation was documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.

Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on a review of a household member?s record and an interview with the provider, the provider failed to ensure that the household member had a search of the central registry within 30 days of becoming 14 years old.

Evidence: The record of household member #2 did not contain central registry search results. The record documented that household member #2 turned 14 years of age on 4/01/2024. The provider acknowledged that the central registry search had not been obtained.
