Inspector Notes

An unannounced monitoring inspection was conducted on November 15, 2023 from approximately 4:10 pm to 5:20 pm. Four staff were present with ten school age children at the onset of inspection. Additional children and staff arrived during inspection. Children were observed participating in various organized group activities. Five child records and five staff records were reviewed. Violations found are noted on the violation notice.

Evidence: The following was found during record review:
1. The record for child #4 (start date 9/5/2023) did not have two emergency contacts other than parents, physician information and a written agreement signed by parents.
2. The record for child #5 (start date 9/5/2023) did not have two emergency contacts other than parents.
