During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on December 2, 2024 and concluded on December 3, 2024. The on-site inspection began at 3:10 pm and ended at 6:00 pm. The inspector reviewed compliance in the areas listed above. There were fifty-one children present and twelve staff. The inspector reviewed five children?s records and five staff records on-site on December 2, 2024 and electronically on December 3, 2024. An additional seven staff records were reviewed to verify background checks. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Submit your POC within five (5) business days from today, which will be the close of business on December 16, 2024. A POC submitted after this date will not appear on the public website.

The center must request a search of the central registry prior to the employee's first day of employment. Staff #1, #2, #3, #9, and #11, who had been employed employed between one month through four months, did not have documentation of date central registry search was requested to demonstrate they were requested prior to beginning employment.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #5 and Staff #7, employed for more than five months, did not have documentation of requesting out of state central registry check and sex offender check for those states.

(Repeat violation) For staff hired before July 1, 2024, centers must obtain the results of a central registry search for an employee, within 30 days of service. Staff #4, #10 and #12 did not have a completed central registry and they have been employed between five months through a year.
