Inspector Notes
A monitoring inspection was conducted on 04/11/2023 between the hours of approximately 11:10 am and 2:50 pm. There were 99 children present with 21 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
91) The record for Child #2 (start date 01/17/2023) did not contain an immunization report.
Evidence:
1) The most recent immunization report in the record for Child #3 (dob 03/09/2022) was dated 07/12/2022.
Evidence:
1) On the date of inspection, the record for Child #2 (start date 01/17/2023) did not contain the results of a physical examination.
Evidence:
1) The documentation shall have been completed within the last 30 calendar days of the date of employment. - The Tb screening form in Staff #2's (start date 03/06/2023) record was dated 1/07/2023.
2) The documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children - The Tb screening form in Staff #1's (start date 01/23/2023) record was dated 03/01/2023.
Evidence:
1) The orientation training log in the records for Staff #1 and Staff #2 did not contain documentation that staff had been trained in prevention of shaken baby syndrome and abusive head trauma, including procedures to cope with crying babies or distraught children.
Evidence:
1) The record for Staff #1 (start date 01/23/2023) and Staff #2 (start date 03/06/2023) did not contain documentation of having completed orientation training in first aid and CPR.
Evidence:
1) The center's director stated that Staff #4 completed orientation training upon hire on 10/17/2014, however, documentation of Staff #4's orientation training was not located in their record.
Evidence:
1) On the date of inspection, the center's director stated that the center had not developed written procedures for prevention of shaken baby syndrome or abusive head trauma.
Evidence:
1) On the date of inspection, the 2022 emergency drills log provided for review, did not have a lockdown drill documented.
