Inspector Notes
A renewal inspection was conducted on 04/02/2024 between the hours of approximately 12:55 pm and 3:00 pm. There were 94 children present, with 19 staff supervising. Children were observed while taking their scheduled rest time, reading books, tracing and coloring.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
3Based on record review, it was determined that the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Sex Offender Registry search request and it was obtained prior to the first day of employment.
1) The record for Staff 3 (start date 03/07/2024) did not contain documentation of an out-of-state Sex Offender Registry search request. Staff #3 had declared in their sworn statement having lived out of the state of VA within the last 5 years before employment.
Evidence:
1) Medication authorization for Child #4 had an expiration date of 01/2024, for Child #5 had an expiration date of 02/2024, for Child #6 had an expiration date of 02/02/2024. Medication was in the center.
2) Medication for Child #3 had an expiration date of 01/2024. Medication was in the center.
Evidence:
1) The attendance record in the Bumblebees class had 5 children recorded in attendance. At the time of inspection, 9 children were present in the classroom.
2) The attendance record in the Purple Finch class had zero children documented in attendance. At the time of inspection, 12 children were present.
