Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/11/2024. The on-site inspection began at 11:00am and ended at 1:50pm. The inspector reviewed compliance in the areas listed above. There were 113 children present and 22 staff. The inspector reviewed 6 children?s records and 7 staff records. This inspection included document review, a tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

REPEAT VIOLATION
SYSTEMIC DEFICIENCY
Based on record review and interview, the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Sex Offender Registry search request and it was obtained prior to the first day of employment.
Evidence: Staff #3s sworn disclosure statement documented that they resided in Florida within the last five years. Center management confirmed that they had not requested a Florida sex offender background search for staff #3.
Plan of Correction: We can get the sex offender search completed today. We'll come up with a system to identify when staff have lived outside of Virginia to make sure the out of state background checks are completed.

Based on record review, the center did not obtain documentation that each child has received immunizations prior to the child attending the center.
Evidence: Child #2s record does not contain documentation of immunizations.
Plan of Correction: I've contacted the family and they said that they have the documentation at home and will provide it today.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. Cleaners were observed in an unlocked cabinet above the sink in the lady bug classroom, the purple finch classroom, the meadow lark classroom and robin classroom.
2. Cleaners were observed on an open shelf in the boys bathroom.

Based on record review, childrens records did not contain all required documentation.
Evidence: Child #4s record did not contain documentation that the childs age and identity has been verified.
Plan of Correction: I've contacted the family and they will provide the proof of identity today.
