During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/21/2024, as a part of the licensure period. The inspection began at 9:25 AM and ended at 11:45 AM. The inspector reviewed compliance in the areas listed above. There were 34 children present and 5 staff. The inspector reviewed 4 children?s records and 3 staff records on site. This inspection included:
- document review,
- tour of the facility,
- interviews, and
- observations

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 31, 2024. A POC submitted after this date will not appear on the public website.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

There shall always be at least one staff person on duty who has obtained within the last three years instruction in performing daily health of observation of children

There was a bottle of copier ink with a warning label including keep out of reach of children was on a windowsill within reach of the school age children in care.
