Inspector Notes

An unannounced renewal inspection was initiated and concluded on November 13, 2023. There were 24 children in care ages four to ten years old with three staff. Children and onsite staff records were reviewed. Children were observed in teacher and self-directed activities including transitions, snack, homework and table top activities. Violations regarding required postings and children's immunizations and physicals were observed and discussed with the on site program director at the conclusion of the inspection.

Based on record review and interview the center failed to ensure that immunization documentation is obtained for each child before the child can attend the center.
Evidence:
1 - There were not any immunization records available for review during the inspection for any of the children in enrolled in the program.
2 - Staff #1 stated that she did not have access to the children's immunization records at the center location.
