Inspector Notes
An unannounced Renewal Inspection took place on 01/10/2023 between the hours of 11:10am and 1:50pm. There were five (5) classrooms observed with a total of 76 children with 13 staff supervising within the supervision guidelines. The children were observed playing with manipulatives, story time, diaper changing, lunch and other organized activities. A complete inspection of the physical plant, administration, children and staff records, evacuation drill logs, medication and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations
5Evidence:
1. On the Toddler-Twos playground, the mulch does not provide a safe fall zone. There is bare dirt under playground equipment (climber and slide) and throughout the play area.
2. Several large tree roots are exposed where the dirt and mulch does not cover.
3. Mulch is less than ?? on most areas of the playground.
Evidence:
1.Child #5 (Infant classroom) was sleeping in a crib that was not labeled.
2. Staff #5 acknowledged the crib was not labeled for Child #5 (start date 08/2/2022)
Evidence: Child #7 in the Toddler Classroom did not wash their hands after toileting.
Evidence: 1. Staff #5 (Toddler classroom) did not use soap and water to clean the diapering surface after each use.
2. Staff #5 only used a bleach solution to sanitize the diapering surface.
Evidence:
1. The Infant classroom failed to sign in Child #6.
2. Staff #5 acknowledge that Child #6 was not signed in upon arrival. Child #6 arrived at approximately 8:35am.
3. At time of observation (11:35am) child #6 was not signed in on attendance sheet.
Violation corrected during the inspection.
