Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on 09/01/2021 and concluded on 09/08/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 67 children present with 16 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of documentation and interview, the center did not ensure that one out of four staff members obtained a copy of a sex offender registry check from any state in which the individual has resided in the preceding five years prior to hire.

Evidence: 1) The record of Staff #2, hired on 08/11/2021, indicated the staff had resided outside of Virginia in the last five years. The record did not contain the results of an out-of-state sex offender registry check from the state the staff member had resided. 2) During interview, a member of management reported the out-of-state sex offender registry check was not obtained for Staff #2 prior to hire.
Plan of Correction: The results were obtained on 09/08/21. In the future, the center will reference the out-of-state contact list and complete the required out-of-state background check searches prior to hire.
