When staff are supervising children, they shall always ensure their care, protection, and guidance.
On October 30, 2025, video footage showed staff #1 leaving the classroom with five toddlers. Staff #1 left child #1 (2-year-old) alone in a classroom for approximately 6 minutes before the child was found by staff. When child #1 was found by staff, the child was not upset or crying.

Plan of Correction: Staff will find time each morning to huddle as a staffing team, talk through the day's transitions and ensure they are doing Name to Face every transition when leaving their classroom including going outside to the playground.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the centers own policies and procedures that are required by the standards.
1. The center?s policy states that ?No child shall ever be left unsupervised for any reason. All staff members are responsible for the children in their classroom and assuring that all children are safe and accounted for; and that staff should be aware of the names and number of children present at all times.
2. Video footage reviewed on November 19, 2025, showed staff #1 leaving child #1 (2-year-old) in the classroom alone for 6 minutes.

Plan of Correction: Previously, staff were required to count children upon leaving and entering new spaces using the "Name to Face" verification only when leaving the center grounds. Our updated protocol now mandates enhanced "Name to Face" verification for all transitions. Every class must now perform a comprehensive "Name to Face" count every time they leave he classroom and every time they enter a new space. This enhanced procedure applies to all transactions, even those involving short distances.
