During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 10, 2026. The on-site inspection began at 9:20am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 98 children present and 24 staff. The inspector reviewed 10 children?s records and 10 staff records on-site electronically on March 10, 2026. This inspection included a tour of the facility, observations, interviews, and document review.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 23, 2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The center's medication policy states that "If medication has expired or a new authorization form is needed, the staff will immediately notify the family in writing as well as management." Child #11 had emergency medication that expired January of 2026, and the family or management had not been notified.
Plan of Correction: A classroom staff will inform the parents of the expired medication and obtain new medication that has not expired. A form has been created and posted in all classrooms showing when medication expires and the classroom where the medication is located. Staff will inform parents of upcoming medicine expiration date prior to expiration.
Standard 8VAC20-780-500-A
Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine.

Staff changed a child?s diaper in the two?s classroom and did not wash the child's hands with soap and water.
Plan of Correction: Staff shall be aware of all items they are touching with gloves, changing gloves in accordance with 8VAC20-780-500. All staff made to review procedures and ensure children's hands are washed after all diapering and toileting.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Medications that are not picked up by the parent within 14 days shall be disposed of by the center.

The medication authorization for Child #11 expired over 18 days ago. The authorization had not been renewed, and the medication was not returned or disposed of as required.
Plan of Correction: A classroom staff will inform the parents of the expired medication and obtain new medication that has not expired. A form has been created and posted in all classrooms showing when medication expires and the classroom where the medication is located.
Standard 8VAC20-780-60-A
(Repeat Violation) Each center shall maintain and keep at the center a separate record for each child enrolled.

The record for child # 8, did not contain documentation of viewing proof of the child?s identity and age.
Plan of Correction: Administrative staff member contacted the parents who brought in a copy of the birth certificate.
