Inspector Notes

An onsite code compliance inspection was conducted October 27, 2023. The inspection was initiated at approximately 3:15 PM and concluded at approximately 4:30 PM. There were eight children in care, ranging from five to 11 years of age. Two staff members were supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of five staff records and interview, the center did not ensure one employee completed background check at least every five years for staff or volunteer.
Evidence:
1.The record for Staff #2 contained a fingerprint based national criminal record check dated 09/19/2018 and a central registry finding dated 11/08/2017.
2. Staff #2 confirmed the information during the inspection.
Plan of Correction: The required background checks were updated.
