Inspector Notes
This inspection was conducted by licensing staff using an alternate protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 04/16/2021 and concluded on 04/22/2021. The director was contacted by telephone to initiate the inspection. There were 13 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 2 staff records and 2 children's records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.
Violations
2Evidence:
1. Children's backpacks and coats were observed placed on the tables and attached seats in the cafeteria during the inspection of the after school program at 4:17 pm on 04/23/2021.
2. Staff 1 stated that the practice of the after school program was to store children's belongings on the tables and attached seats.
*A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.
Evidence:
1. The written communicable disease statement in the record of child 1 is incomplete and does not include all required information.
a. The statement in the enrollment record reads as follows:
"I agree to inform the YMCA child care staff/director within 24 hours of or the next business day by the State Board of Health, except life threatening diseases which must be reported immediately.
2. The statement lacks the following required information:
...."after his child or any member of the immediate household has developed any reportable communicable disease...."
