During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 10/22/2025, as a part of the licensure period. The on-site inspection began at 7:10 AM and ended at 8:35 AM. The inspector reviewed compliance in the areas listed above. There were 23 children present and 4 staff. The inspector reviewed 8 children?s records on site and 4 staff records electronically on 10/27/2025. This inspection included:
? document review,
? tour of the facility,
? interviews, and
? observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/11/5/2025. A POC submitted after this date will not appear on the public website.
Violations
4Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening that was obtained within 30 days prior to employment.
The date of employment of staff 2 was 9/4/2025 and results of a tuberculosis screening were not obtained until 9/14/2025.
The thermometer in the first aid kit was broken.
An injury record dated 10/17/2025 included only the parent signature.
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the names, addresses, telephone numbers and places of employment for each parent.
The record provided for child 7 did not include any information regarding the father.
