Inspector Notes

An unannounced monitoring inspection was conducted on 05/05/2022. There were 13 children, ages 3 years-5 years, and 2 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed arriving to the program, during circle time, in music and movement and having a morning snack. A total of 5 children?s records and 2 staff records were reviewed. The inspector arrived for the inspection at 8:30 am and departed at 9:45 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation, measurements and discussion with staff, the center failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards described in the American Society for Testing and Materials and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

Evidence:
1. The height of the slide on the plastic jeep play structure measured 1 foot 11 inches high, which requires a 5 foot 11 inch fall zone for the slide trajectory. There was only 3 feet of resilient surfacing, creating the fall zone, for the slide trajectory. The fall zone for the slide did not comply with the minimum safety standards described in the American Society for Testing and Materials.
2. There was only a 4 foot 4 inch fall zone on the side closest to the fence of the dome climbing structure. The fall zone for the dome climbing structure did not comply with the minimum safety standards described in the American Society for Testing and Materials.
Plan of Correction: Will discuss with facilities manager and have it repaired.
