Inspector Notes

An unannounced monitoring inspection was conducted on-site on 11/21/2022 and concluded with an electronic record review on 11/23/2022. There were 7 children, ages 3 years-4 years, and 2 staff members providing direct supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies and special services. The children were observed during arrival from field trip, story time and meal time. A total of 5 children?s records, and 2 staff records were reviewed. The inspector arrived for the inspection at 11:30 am and departed at 12:30 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation noticed issued to the provider.

Based on observation, measurements and interviews with staff, the center failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards described in the American Society for Testing Materials and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
Evidence:
1. The height of the slide on the plastic jeep play structure was measured at approximately 1 foot 9 inches, which requires a 5 foot 9 inch fall zone for the slide trajectory. There was approximately 3 foot 10 inches of resilient surfacing, creating the fall zone for the slide trajectory.
2. There was approximately a 4 foot 3 inch fall zone on the side closest to the fence of the dome climbing structure. A 6 foot fall zone is required.
Plan of Correction: Staff will consult administration.
