Inspector Notes

An unannounced monitoring inspection was conducted on 3/27/2023 from 10:25 am-11:15 am. There were 9 children (27 points) directly supervised by 2 caregivers. The home, 2 caregiver records, 1 household member record, 5 children?s records, evacuation drills, and policies were inspected. Areas of non-compliance are identified in the Violation Notice. Based on record review and interiew, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: There was no documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance for child #4. The date of enrollment for child #4 is 8/23/2021.
Plan of Correction: I will ask the parents today to have the doctor fill it out.
Standard 8VAC20-800-240-A
Based on observation and interview, furnishings inside the family day home were not maintained in a safe condition.

Evidence: Five rectangular children's play vinyl mats and a circular vinyl ball pit were torn, and cracked in various places, exposing the foam within.
Plan of Correction: I will remove them today until ordered.
