An unannounced renewal inspection was conducted from 11:30 am ? 1:30 pm with the provider and two assistants. There were ten children, ranging from two months to five years of age, for a total of 26 points. Children were observed preparing for and eating lunch and napping. Lunch served today: turkey meatloaf, candied beets, green beans, rolls and milk. Three children's records, three provider records, the emergency drill log, daily attendance, and annual training were reviewed. The first aid kit, battery operated flashlight, weather band radio and carbon monoxide detector were observed and in compliance. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care. Standard 8VAC20-800-100-A
Based on a review of three children's records, the provider did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within 30 days after attendance. Evidence: the record for child #3 (date of first attendance: 3/21/22) did not contain documentation of a physical examination.
Plan of Correction: Child's parent was contacted during the inspection and documentation of the physical was requested.
Standard 8VAC20-800-680-A
Based on observation, not all caregivers washed their hands as required. Evidence: caregiver #2 was observed changing a diaper and then helping two children into a high chair before washing their hands.
Plan of Correction: Will retrain all caregivers on diapering and handwashing procedures.
Standard 8VAC20-800-680-B
Based on observation, caregivers did not ensure that all children?s hands were washed with liquid soap and warm running water as required. Evidence: after an infant had a diaper change they were placed directly into a play area without washing their hands. A second infant was observed being fed a bottle and did not have their hands washed beforehand.
Plan of Correction: Will retrain all caregivers on diapering and handwashing procedures.
Standard 8VAC20-800-690-D
Based on observation and interview, the provider failed to ensure that the diapering surface was cleaned and sanitized as required. Evidence: after an infant was changed the diapering surface was not cleaned and then sanitized as required afterwards.
Plan of Correction: Will retrain all caregivers on diapering procedures.
