Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 11-7-2024 and completed on 11-13-2024. The on-site inspection began a 12:05pm and ended at 3:25pm. The inspector reviewed compliance in the areas listed above.
There were 63 children present and 14 staff. The inspector reviewed 6 children?s records and 6 staff records on-site and electronically on 11-12-2024. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11-25-2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. On the "older" playground there were areas of rust, chipping and peeling paint on the metal playground piece.
Plan of Correction: A work order was submitted to have the equipment repainted.
Standard 8VAC20-780-350-B-1
The required ratio for children ages birth up to 16 months is one staff per 4 children.
1.Staff #7 was observed alone in the classroom supervising 8 children , the youngest child, child #2 was age 10 months old. Staff #7 was alone for approximately an hour.
2. Staff #6 was observed alone in the classroom supervising 5 children, the youngest child, child #6 was age 15 months old. Staff #6 was alone for approximately an hour.
Plan of Correction: With clarification regarding the resting time ratio for children under 16 months,
management adjusted the staffing schedule to ensure that the 1:4 ratio is
observed at all times for this age range. Corrected on 11-8-2024
Additionally, the designated class for 14-18 month olds wil be adjusted so that only children at least 16 months old are enrolled in the class at any given time so that the 1:10 resting period ratio standard is followed.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. This list shall be dated and kept confidential in each room or area where children are present. Child #3 has milk sensitivity, Child #5 has a nut allergy and Child #6 has an egg allergy and their names were not written on a current list in their classrooms. These 3 children are in sperate classrooms.
Plan of Correction: Staff in these classrooms were reminded that a current list of allergies, sensitivities, and dietary restrictions must be maintained and updated monthly. Missing lists were posted immediately
Standard 8VAC20-780-500-B
The diapering surface shall be used only for diapering or cleaning children, and it shall be cleaned with soap and at least room temperature water and sanitized after each use. Staff #8 was observed diapering a child and did not wash or sanitize the diaper pad.
Plan of Correction: The staff member was reminded about the proper procedures for cleaning
and sanitizing the diapering surface after each use.
