During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 4/1/2025 and completed on 4/9/2025. The on-site inspection on 4/1/2025 began at 11:25 am and ended at 2:25 pm. The inspector reviewed compliance in the areas listed above. There were 71 children present and 17 staff. The inspector reviewed 7 children?s records and 7 staff records on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/17/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-500-A
Staff shall wash hands with soap and running water before and after a diaper change and children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Staff did not wash their hands after changing a child?s diaper and did not wash the child's hands after the diaper change.
Plan of Correction: The staff member was reminded of the
proper handwashing procedures before &
after a diaper change on 4/02/2025.
Additionally, retraining on proper
handwashing procedures was provided to
all staff between 4/02/2025-4/10/2025.
Standard 8VAC20-780-500-B
(Repeat Violation) The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. Staff did not clean or sanitize the diaper changing surface after a diaper change.
Plan of Correction: The staff member was reminded of the
proper cleaning & sanitizing procedures
for the diapering surface on 4/02/2025.
Additionally, retraining on the proper
procedures for cleaning & sanitizing
was provided to all staff between
4/02/2025-4/10/2025.
Standard 8VAC20-780-530-A-1
At least one staff in each classroom or area where children are present shall have current certification in CPR and shall include an in-person competency demonstration. The staff with a group of twelve, two-year-old children, had not completed the in-person competency demonstration required.
Plan of Correction: With 17 of 23 staff members currently
certified in CPR & First Aid, management
will ensure at least one certified staff
member is present in each class daily. CPR
& First Aid certification training has been
scheduled on 4/30/2025 for 3 additional
staff members.
