The trashcan in Room E had areas of flacking rust.

Repeat Violation
When an authorization for medication expires, the facility shall notify parents the medication must be picked up within 14 days of the authorization expiration or that a new authorization should be completed. Child #1 has 3 medication authorization for 3 medications, and the authorizations were overdue by more then 2 weeks. The authorizations had not been renewed, and the medications had not been returned or disposed of as required.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The attendance sheet for Classroom A was not accurate as it listed 7 children in care and there were 9.
