The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #1 employed for more than 6 months did not have documentation of requesting a central registry or criminal record background check from one state. Staff #2 employed for more than one year did not have documentation of requesting a central registry or criminal record background check from a state.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. In an unlocked cabinet under a sink in the classroom, there are more than 10 bottles of hand sanitizer within reach of children.

Cosmetics, medications, or other harmful agents shall not be stored in areas, purses or pockets that are accessible to children. In a classroom where children were present there was a purse resting in a chair within reach of children.

Provision shall be made for an individual place for each child's personal belongings. In after school child care room there is not an individual space for each child?s personal belongings.

Rest mats or beds shall be provided for children during the designated rest period. There were not any rest mats available for children during the designated rest period.

Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. Children did not wash their hands with running water or disposable wipes after eating snack.
