Inspector Notes

An onsite code compliance inspection was conducted October 12, 2023. The inspection was initiated at approximately 10:20 AM and concluded at approximately 12:20 PM. There were 14 children present, ranging from three to five years of age, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-A
Based on review of five staff records and interview, the center did not ensure three employees completed background checks were obtained at least every five years for staff or volunteer.
Evidence:
1. The record for Staff #1 (Date of employment 11/04/2013) contained a fingerprint based national criminal record check dated 01/29/2018.
2. The record for Staff #6 (Date of employment 09/02/2004) contained a fingerprint based national criminal record check dated 01/30/2018.
3. The record for Staff #7 (Date of employment 02/08/2018) contained a fingerprint based national criminal record check dated 02/08/2018.
4. Members of management confirmed as of 11/28/2023 the required updated background checks had not been obtained.
Plan of Correction: We will ensure background checks are obtained within the required timeframe going forward.
