Inspector Notes

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 12, 2023 from approximately 10:30 AM to 12:30 PM. There were 14 children in attendance and a total of two staff supervising. A total of five children and seven staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-550-2-f
Based on a review of seven staff records and interview, the vendor did not obtain documentation of subsequent background checks every five years for three staff members.

1. The record for Staff #1 (Date of employment 11/04/2013) contained a fingerprint based national criminal record check dated 01/29/2018.
2. The record for Staff #6 (Date of employment 09/02/2004) contained a fingerprint based national criminal record check dated 01/30/2018.
3. The record for Staff #7 (Date of employment 02/08/2018) contained a fingerprint based national criminal record check dated 02/08/2018.
4. Members of management confirmed as of 11/28/2023 the required updated background checks had not been obtained.
Plan of Correction: We will ensure all required background checks are obtained for staff.
Standard 8VAC20-790-560-A
Based on review of seven staff records, the vendor did not ensure one staff was evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the required timeframe.
Evidence:
1. The record for Staff #2 (Date of Employment 06/25/2023) contained a TB screening dated 09/26/2023.
2. Members of management confirmed this information during the inspection.
3. The documentation of the TB screening is required to be obtained within the last 30 calendar days of the date of employment and be signed by a physician, physician?s designee, or an official of the local health department.
Plan of Correction: Going forward, we will ensure all new staff have the TB screening within the required timeframe.
Standard 8VAC20-790-600-C
Based on review of seven staff files and interview, the vendor did not ensure orientation training for three staff was completed prior to the staff member working alone with children and within seven days of the date of employment.
Evidence:
1. The records for Staff #2 (Date of Employment 06/25/23), Staff #4 (Date of Employment 02/22/23), and Staff #5 (Date of Employment 06/13/23) did not contain documentation of orientation.
2. A member of management stated orientation was completed but all the required topics were not covered.
Plan of Correction: We will use the model orientation forms to ensure all required topics are reviewed.
