Inspector Notes

An unannounced , on-site monitoring inspection was initiated on 12/09/2024 and completed on 12/09/2024. The on-site inspection began at 3:22 pm and ended at 5:40 pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and 5 staff. The inspector reviewed 5 children's records and 3 staff records on-site on 12/09/2024.

Standard 22.1-289.035-B-2
Submit to fingerprinting and provide personal descriptive information described in subdivision
B 2 of ? 19.2-392.02; The results of the fingerprint background check shall be obtained
prior to employment.

The results of the fingerprint background check for staff 1 were obtained more than 30 days
after her first day of employment.
Plan of Correction: Administrative staff stated that all background checks will be obtained prior to hire going forward.
Staff 1 was hired as a substitute and it took until October to receive her updated fingerprints.
Standard 8VAC20-780-160-A
Repeat Violation
The documentation of the negative tuberculosis screening shall have been completed within the last 30 calendar days of the date of employment.

The TB screening for staff 1 was completed more than 90 days prior to employment.
Plan of Correction: Administrative staff stated that all staff will obtain a TB screening prior to hire
and to have been completed within 30 days.
Staff 1 - had been a sub. in school and I thought the TB training would be fine despite moving over to Social Services. She was TB screened in April because she was substituting.

Repeat Violation
The Virginia Department of Education sponsored orientation course shall be completed within 90 calendar days of employment.

Staff 1, who has been employed more than 90 days, has not completed the Virginia Department of Education sponsored orientation course.
Plan of Correction: Administrative staff stated that staff 1 is working on this training and will have
completed as soon as possible.
Staff 1 continues to work on her training.
Standard 8VAC20-780-240-D
Prior to working alone with children and within seven days of the first day of employment, staff
shall be provided in writing with the center's information listed in 8VAC20-780-420 A and
the policies required by 240.D.1-7 and 240.E.

There was no written documentation to demonstrate that staff 2 has completed the
required orientation training. Staff 2 has been employed for more than 7 days.
Plan of Correction: Administrative staff stated that orientation training is in process for staff 2. Going forward all new staff will complete orientation training within 7 days as required.
Staff 2 completed her training within the 7 days, unfortunately the paperwork was not in the file.
Standard 8VAC20-780-40-D
The license shall be posted in a place conspicuous to the public (? 22.1-289.011 of
the Code of Virginia).

The current license was not posted in an area that is visible or conspicuous to the public.
Plan of Correction: The license will be posted in the library area, near the front door, of the building located at 418 Colonial Avenue.

Repeat Violation

The following nonmedical emergency supplies shall be required: One working, battery operated radio in each building used by children.

There was no battery-operated radio available in the building located at 419 Colonial Avenue.
Plan of Correction: Administrative staff stated that a battery operated radio would be obtained for the building located at 419 Colonial Avenue.
Ordered - We will put 418 one in bag until new one for building arrives.
