Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on 3/31/2021 and concluded on 4/27/2021. A complaint incident was received by the department regarding allegations in the area of Supervision. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation. Based on a review of video footage, review of records, review of policies and procedures, and interviews, the evidence gathered during the investigation did not support the allegation of non-compliance with standards or law. Any violations identified during the course of the investigation can be found on the violation notice.

1.The injury record dated 03/26/2021 for Child #1 contained one staff signature. The injury record did not contain a parent signature or a second staff signature as required.
