Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 5/11/2021 and concluded on 5/11/2021. The director was contacted by telephone to initiate the inspection. There were 26 children present and 4 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 staff records, and backgrounds for all agents submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center were not obtained for one child.

Evidence:
1. The first date of attendance documented in the record for Child #1 was 03/22/2021.
2. The documentation of immunizations in the record for Child #1 was dated 04/13/2021.
Plan of Correction: The immunizations for Child #1 were obtained and placed in the record. I will remind parents of the requirement to provide documentation of immunizations prior to starting when I give tours. It is also listed on our enrollment checklist.
