Inspector Notes
An unannounced monitoring inspection was conducted today from 9:26am-12:00pm to determine compliance of the facility while they operate under a provisional license. There were 39 children directly supervised by 9 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in being bottle fed, building with blocks, doing puzzles, and playing outside. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

9Evidence:
Staff 1, 2's records did not contain documentation to demonstrate that the staff members have fulfilled all requirements to be qualified as a program lead. Both staff were the only caregivers in the classroom during the inspection.
