During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 11/18/2024 and completed on 11/18/2024. The on-site inspection began at 2:00pm and ended at 3:10pm. The inspector reviewed compliance in the areas listed above. There were 54 children present and 10 staff. The inspector reviewed 6 children?s records and 5 staff records [on-site].
This inspection included: document review, tour of the facility, interviews, and observations.


Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/27/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

While playing on the playground two school aged children jumped over the fence and ran in the shared parking lot and in front of the dumpster before jumping back over the fence into the playground.
Staff 2 was observed on the playground but did not intervene when the children left the playground.
Plan of Correction: Review again playground
safety and supervision

with the staff member, along

In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

On the date of inspection, Staff 2 was observed as the sole staff member supervising children on the playground. Staff 2's record does not contain documentation that they meet the qualifications for a program leader.

At least one staff in each classroom area where children are present shall have current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council.

Staff 2 was observed as the only staff member supervising children on the playground. Staff 2's record does not contain documentation of a CPR certification.
