Inspector Notes
An unannounced inspection was initiated on May 19, 2025 and completed on May19, 2025 in response to a complaint received by the licensing office on May 15, 2025 relating to allegations of safety of the physical plant. The inspector reviewed 1 child?s records and 1 staff record on-site.
This inspection included document review, tour of relevant areas of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on May 27, 2025. A POC submitted after this date will not appear on the public website.
Violations
1On May 5, 2025, while transitioning indoors under staff supervision, Child 1's finger was caught in the hinge jamb of the exterior door. Child 1 sustained a laceration injury requiring outside medical treatment.
In addition to the maintenance of the door, staff were spoken to regarding the closing and opening of doors. Teachers have been reminded to always open and close doors for children whether it?s for restrooms, classrooms or entrance/exit doors, and to not allow the child to open or close doors. The Director has also put in place a `line starts here? sign in all classrooms and lobbies to show children the safest place to line up when in front of a door. This allows for a safe distance between children and doors that are being opened or closed. The door adjacent to the playground nearly always remains open while children are on the playground/outdoors. It may be closed in the event of any storm or emergency situation. If and when this door is being closed or opened, the `line starts here? sign will assist in creating a safe distance from the door and where the children stand in line.
