Inspector Notes

An unannounced renewal inspection was completed on January 31, 2024. Inspector arrived at 8am and departed at approximately 11am. There were 65 children and 13 staff members present. The Inspector observed five children?s records and eight staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during indoor and outdoor play, group activity, breakfast, handwashing, bottle feeding, and diaper changes. Emergency preparedness documents were observed. Medications are administered at the center and were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented. The violation is listed on the violation notice issued to the center and were reviewed with the Director at the exit interview. The inspection concluded on 1/31/24.

Based on record review and interview, it was determined that the center did not ensure that they obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #2 (date of hire 12/28/2023) disclosed and signed on their sworn statement on 12/28/23, that they lived outside of Virginia the past five years. There was no out-of-state background check completed for Staff #2.
2. Staff #3, disclosed and signed on their sworn statement that they lived outside of Virginia the past five years and also has an out of state identification card. There was no out-of-state background check completed for Staff #3.
3. Staff #1, Director, confirmed that Staff #2 and Staff #3 do not have their out of state background checks in their record.
Plan of Correction: The center responded with the following: The center will process Staff #2 and Staff #3 out-of-state background checks immediately and will send the results to the DOE.
