During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on January 9, 2025. The on-site inspection began at 12pm and ended at 3pm. The inspector reviewed compliance in the areas listed above. There were 61 children present and 14 staff. The inspector reviewed 6 children?s records and 5 staff records on-site on January 9, 2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.057-A
The program is required to submit to VDH and implement a plan to test potable water for lead. The program did not have evidence of a testing plan submitted to VDH and/or the program did not have evidence of the implementation of the required testing plan.
Plan of Correction: The center responded with the following: The center will get water lead testing completed and submit their testing plan to VDH and DOE. Information will be sent via email to the inspector as well.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Both outdoor playgrounds did not have the required resilient surfacing. There were patches of dirty throughout the playground with minimal mulch under the fall zones. The mulch was measuring about 1 inch in depth.
Plan of Correction: The center responded with the following: The center will speak with the board members to discuss resilient surface. The center will look at getting more mulch to both playgrounds. The center will inform licensing the plan for outdoor resilient surfacing.
Standard 8VAC20-780-60-A-8
A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #1 and Child #2 both have a documented food allergy on their physicals and there were no allergy action plans in their record.
Plan of Correction: The center responded with the following: The center will obtain Child #1 and Child #2?s allergy action plans from their pediatricians and place them in their files and medical binder.
Standard 8VAC20-780-70
For staff hired after March 1, 1996, documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. Staff #1, Staff #2, Staff #3, and Staff #4 did not have two or more references on their staff record and were present working with children.
Plan of Correction: The center responded with the following: The center will obtain all references for the staff members and ensure that all staff members have references prior to the first day of employment.
