During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on December 9, 2024 and completed on December 9, 2024, as a part of the current licensure period. The on-site inspection began at 8:30am and ended at 10:00am. The inspector reviewed compliance in the areas listed above. There were 44 children present and 13 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #3, had an initial criminal background check request from previous state of residence, dated 48 days after date of hire.
Plan of Correction: Going forward, all new staff will have any required out-of-state background checks requested prior to date of hire.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening that has been completed within the last 30 calendar days of the date of employment.
Plan of Correction: Going forward, we will have new staff complete TB screening within 30 days prior to date of hire.
Standard 8VAC20-780-240-B
Staff shall complete orientation training prior to working alone with children and no later than seven days of the date of assuming job responsibilities.

Staff 3 had orientation training documentation completed 48 days after date of hire.
Plan of Correction: Going forward, new staff will complete orientation with the first 7 days of hire date.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

Cords from the telephone were hanging within reach of the children in the Cardinals classroom.
Plan of Correction: Cords secured to wall while inspector was here.
Standard 8VAC20-780-290-A-2
Fans, when used, shall be out of reach of children.

Fans were in three classrooms, within reach of children.
Plan of Correction: Fans will be moved so as not to be within reach of children.
Standard 8VAC20-780-550-D
The center shall implement a monthly practice evacuation drill.

There was no documentation of a fire drill being done in November 2024.
Plan of Correction: We will immediately document emergency drills when performed each month.
