Inspector Notes

An unannounced, on-site monitoring inspection was initiated on August 20, 2024 and completed on August 23, 2024, as a part of licensure period. The on-site inspection began at 12:56 pm and ended at 4:10 pm. The inspector reviewed compliance in the areas listed above. There were 99 children present and 11 staff. The inspector reviewed 9 children's records and 6 staff records onsite. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or
law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Based on review of staff records and interview, the center did not ensure to obtain all background checks within the past five years.

Evidence: In Staff#3's record (date of hire: 05/01/21) the last dated central registry was 07/27/18 and results of fingerprint based background check is 05/21/18.

Based on observations, the center did not ensure all hazardous substance were kept in a locked place using a safe locking method that prevents access by children.

Evidence: 1. In the Youth watch room, there was a bottle of disinfectant cleaner labeled observed on the top of the cabinet with the labels " keep out of reach of children" and "caution" with the door unlock and accessible to children in care.
