During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An announced monitoring inspection was initiated on March 7, 2025 and completed on March 17, 2025 in response to a self-report received by the licensing office on March 4, 2025 relating to staffing and supervision. The inspector reviewed 1 child record and 3 staff records electronically on March 10, 2025 and March 13, 2025. This inspection included document review, interviews, and observations
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 21, 2025. A POC submitted after this date will not appear on the public website.
Violations
4On February 28, 2025, Child#1(6 years old) was left unattended on the center?s bus during the afterschool program. Child#1 was not supervised by staff for approximately 11 minutes. Staff#3 discovered Child#1 was not accounted for at the center and initiated a search for Child#1. Child#1 was found asleep on the bus by Staff#4.
On February 28, 2025, Staff#1 was the assigned rider to supervise the school age children on their center's bus transport. Staff#1 did not meet qualification of a program leader.
On February 28, 2025, Child#1 (6 years old) was without actual sign and sound supervision of staff for approximately 11 minutes.
The licensee did not ensure the center?s own transportation and supervision policies and procedures were followed on February 28, 2025, when a six year old child was left unattended on the center?s bus for approximately 11 minutes.
